Pretty clear that the problem here is the streamer plugin.. Download it, and add it to the server.cfg. Also, the newest streamer is designed for 0.3x, consider updating your server too.
You stated that the plugins were all good, well, eeh.. nope. They aren't.
The streamer plugin didn't load succesfully. The only thing I can think of is adding the streamer plugin, once again.
Also, use [ code] [ /code] tags the next time you post a server log. Looks a lot better
