Pretty clear that the problem here is the streamer plugin.. Download it, and add it to the server.cfg. Also, the newest streamer is designed for 0.3x, consider updating your server too.
You stated that the plugins were all good, well, eeh.. nope. They aren't.
The streamer plugin didn't load succesfully. The only thing I can think of is adding the streamer plugin, once again.
Also, use [ code] [ /code] tags the next time you post a server log. Looks a lot better

This is your problem. You don't add .dll when on Windows. You only add an extension (.so) when on Linux. It should just be this:

Make sure you put the newest version of streamer and add all plugins to the server.cfg, and don't put extensions because you're on windows.

Are you running on 0.3x?

Replace new plugins and replace includes with latest includes and use only those filterscripts which have both(.amx and .pwn) files and replace samp,announce and etc.. .exe file with new .exe file then try to open.
