Okay as the title says anytime someone leave ID 0 gets teleported I have checked OnPlayerDisconnect But I don't think there is anything out fo the ordinary there. I am just wondering how this happens and how it can be fixed?

Look at loops, timers (especially playertimers) and variables that represents playerid, make sure you reset them when the player leaves.
If none of these worked, try connecting one NPC, when you start the server, this ID will have ID 0
