You need to create a MYSQL database, once you do you put in the information used to create the database into the forum so it can install the needed tables into the database.

I already made a MYSQL Db but i don't understand what i have to put...

Put the MYSQL username in username,
MYSQL password in password,

You see where it says server name: localhost replace that localhost with MySQL host name
