The script uses mysql R5 or R6 and you loaded R34.

I'd still recommend you to learn how to use threaded queries and update both plugin and script to R39-3.

Does Your Host Supports MYSQL ?
If Yes Then Did you create Table On PHPMYADMIN ?

recompile script with latest MySQL plugin, or use the mysql plugin which matches the script version.
