Is the data in the mysql db already (the table structures etc) as that can also be an error even if its not shown on the startup. in addition to this why not try update to the latest versions of blueg's like r34 (most compatible version he released)

switch the mySQL to R39 https://github.com/pBlueG/SA-MP-MySQL/releases

Than provide any errors from server_log and mysql_log
