ok...
1. try use a print and try copy it directly into the sql table in phpMyAdmin and check if you get any error...
2. if it works check if mysql_query and format gets executed, because i've seen some strange bugs sometimes where the functions doesnt get executed...(it sounds like that since you cant find anything in mysql_log)
