That include is not found in your includes file . Be sure that there is a file called "my" in your includes file .

Check it out , if there is alredy a file called "my" in that folder , then put that "my" file in a include folder on Main Menu

His problem is that he can't find it and then include it. I had a re-search for that include and it was like it never exists.
(Its name should be different M... Y... (my))
Ask the person who gave or got that script, where you can find it.
