Turn all details to low, get a smaller resolution - or, get a new graphics card as stewie said.

I also have a 1920x1080 res and all my settings are on medium, with max draw distance.

You don't have to play at 1080p resolution. Lower your resolution if necessary. Aside from that, turn down your video settings or get a better graphics card, as the others said.
