How do you make SA-MP skins?

I mean how to make a skin from scratch, and not just edit colors, clothes etc.

3ds Max + dff plugin, and alot of practice.

you need to know 3D editing quite alot.. and really much experience is needed! (for doing it from scratch)
Otherwise you can follow some tutorials and edit it more easily

One more thing, when I use Photoshop to edit the skin, and type a text, the text spawns flipped in-game, only colors spawn correctly.

Reason is simple cuz the image you editing isn't just an image. It is a texture that is applied to a 3d model. It isn't like a direct overlay. You need to find parts of the image, there position ingame on the model and then edit it.

I just don't know how to explain it correctly because I only did some casual 3d works nothing serious. And as far as skin editing goes, only did it by hit and trial mostly

You don't, you make skins for GTA:SA (obviously they need a proper bone structure), and apply it through SA-MP but SA-MP doesn't have it's own "skin" system.
