That's because the amx file always gets deleted once you have an error, just fix that error.

He doesn't have errors, note, scripts are not plug & use, you must get the includes, etc.

I HAVE EXPLAINED THE PROBLEM.
You don't have the include specified in the error, it says:
"cannot read file from: 'INCLUDE'" where the include is, there's something else meaning that what's written in there is an include and since you don't have it you are getting this error, anyways download that include and place it in the includes folder after that, compile and everything should work.
Not replying again btw, this is a simple problem with a simple fix, I gave you the solution plus some googling would be on your side for attempting to fix this problem.
