Did you upload the .amx file to the gamemode folder?
Did you specify the name of the gamemode in server.cfg?

This is clearly hosted on Volt Host, by Dedicated he means the statement within the console.

OT. You seem to be missing the required plugins (or correct version) that're used by the includes within your script. As posted above, did you specify the required plugins within the server.cfg file along with the correct .so extensions?
