Does this help at all? I know it's similar to what you want, but I think you would have to "hard-code" everything into the PHP script. I don't actually know how to "integrate" an RCON console into a PHP script like they do at Volt-Host or ServerFFS. I'll ask my developer though, he's good at that stuff!
