I have played with all of the NVidia settings and no difference. I just did another fresh reinstall and wiped all settings etc. and it's still the same problem. I don't understand how I've played the game for years without this problem and all of a sudden without me changing a THING I'm suddenly getting this shit..
This is my CPU usage while just idling on the Desktop with nothing running apart from background stuff.
Why am I getting sudden spikes in CPU usage?

It seems my CPU is reaching maximum at times? Why would this be?
EDIT: I've been mainly focusing on the Frame Limiter and NVidia V-Sync settings. It seems that turning both off results in tearing, as expected, but it eliminates most of if not all of the stuttering.. But I can't play with screen tearing..
