To remove city selection, find the arrays created for it and remove them. Or when i read through your post, i think it would be better if you make something complete new. Check samp wiki for the work of arrays, they can help you for your spawnpoints, also for the spawning at the different hospitals make gangzones, or just check if the player is somewhere there in a range.
