"... VALUES('%s', '%s', %s, '%s' '%d-%d-%d)", GetName(playerid) ..." - you forgot a comma before the date and you also forgot to close the apostrofe at the end of the date.

It should be: "... VALUES('%s', '%s', %s, '%s', '%d-%d-%d')", GetName(playerid) ..."
