Log into your account, and click support. At the bottom, there is a link that says "Open a new ticket". Click it, and just tell them that you want them to set up MySQL for you. They usually do it within a few hours, depending on availability of their staff. You will receive an email when it's done, telling you all the connection info and passwords etc.
