You had a few too much closing brackets, replace this OnDialogResponse with yours and it should be fine:

I've redone the 'whole code', just copy and paste this and it should work ( I'm not sure tough, because as far as I know you can just have 20 listitems per dialog but if so you should change that yourself. ). Here it is:

I don't have time to copy the whole code, so i just copied the 1st listitem, (i use this method on mine too), Should work, otherwise try changing your dialog id, might be some conflict with other dialog id's, also in your fs's try changing the return of OnDialogResponse to 0
