My suggestion's on how to do this. (note this is only a suggestion so if you want i can help you do this.

Make a little store anywhere on your map with an interior, Create a Dynamic CP in that store. Create a dialog or w.e to Buy the car insurance. Set a timer so that every whatever minutes you would to be charged. Use a Variable for this too i suggest.
