Hmm I suggest you to take a look at OnGameModeInit callback !
Can you see what's the problem ? I can :P

When you add a menu item ongamemodeinit call back you have to set its real name.
Like, take a look at AddMenuItem, you have 'teleportmenu' everywhere :P Lol sorry if you didn't understand i couldnt explain it better.

P.S I always make that mistake so don't worry lol, it just happens when you copy/paste all of them at the same time.
