Something completely unique...
A speed-o-meter that wouldn't top 130 MPH in normal cars you would usually see down your street in real life for example, Most of the time when i see a speed-o-meter Your in a moonbeam or something really bad like that and your hitting 200MPH when it would really hit rarely 110MPH and thats not very roleplay.

Toilets.
if(ToiletNeeds[playerid] += 50)
You need to go to the toilet and if it reachs 90 you may try to wet yourself which you can prevent yourself from doing, and if it reachs 100 your bladder will burst and you lose alot of health.

Rofl, would be fun if lots of DM nooblets come in and like lose health and like "%s bladder just exploded!" would spam.

Bots that can be controlled and can fight/lose health. Then a way to determine which way the player's camera is facing. Then a way to attach anything to anything (i.e. vehicles to people, vehicles to vehicles, people to ropes)
