hello guys,
i would like to say that i have made a form but the problem is i dont know how to add a option type form can you guys help me ?

You can use JotForms instead.
Make the form then just add the code. Way more easier.
I have used it myself.

People posted tons of tutorials here. For a working example just check the source code of any website with a form. It really isnt that hard to create simple websites in html.
