Team Choosing Box at Connection [SCRIPTING?] - MrGtasagamer - 15.02.2015

Hey,
I wont a dialog box, that comes if I join the server, there I should choose the team, and if I have choosen it, I should choose my skin.
I tried all the ways, but I did not learned how to make it.
I hope someone can help me.
