Sorry my English bad my server lag where online 300 peoples.
Simptomy lags: Beside some people small fps. Car can appear, straight before you.
my server cfg
onfoot_rate 60
incar_rate 60
weapon_rate 60
stream_distance 200.0
stream_rate 2000
CPU Use 50-55% where 300+ online
when was stream_rate 3500, fps became more but car too to appear straight before you.
Why lag server?

Bad coding, check if you have timers creating this lagg.
Improve your script or try to upgrade your hardware of your server.

Probably Host Issue. 300+ Players Hmmm Try to Optimize your script if that dont work out you will need to upgrate your server hardware especially server`s Memory and cpu.
