Server Crashing - kiril7897 - 27.03.2013

Hello Everyone !
I have RolePlay Server
And I have report system in my server
Now Im need help,the problem is :
Player do /report (Bla Bla Bla)
/tr (Trash report)
/ar (Accept report)
Im do /tr it working
but if I do /ar
it not do this and my server Crashed and Closed/

Please tell me how to fix it or send here Message how to fix it
