i replaced the whole server files with fresh ones but still is doing this what is it?

U added plugins lines on server.cfg ? It says sscanf inst it sscanf2 ..?

Put sscanf.dll in the plugins folder.

You need latest version of you all plugins and re-compile your script in order to solve your problem.
