This is a input dialog,cozzz this is a register type of dialog you would have this under OnPlayerConnect.

Where ever you're showing your registration and log in dialogs, create the boxes in a textdraw.

Its made by using Dialog Style Input.

Btw hide the server name using paint or something to hide because its a type of advertising server name
