Do you have the tables in that DB?
I have the same problem using the same mysql plugin...
Server start, connect to db, no problems, but when i connect to server and put password for register server crash cuz i don`t have the tables created where the server should store de info...

Search for the dump.sql for this plugin, i didn`t find it, if you find please PM me.

And use this .inc and change yout sampmysql plugin, that`s the reason.

Use the .inc that i uploaded...
i changed the .inc and the plugin with thoose 2 and the server stoped giving me that errors..but now i`m stuck at the DB cuz i don`t have the dump.sql with the tables...

P.s now i see that you use windows 7 x64... i don`t think they have support for windows x64 the plugin is quite old and they say "win32" ... that ever . test this plugin -> http://www.megaupload.com/?d=9FHR18GL
