From looking at your CPU usage, try running GTA 4 on high priority.

I have closed all background process. Is there any mod or patch that can help tweaking fps?

Wasn't there a program to reduce lag because of a bug in GTA IV?

Do you even have a videocard? I mean, HD4300 and 4500 as far as i know are built-in MB (integrated) videocards. You can't play GTA IV at good FPS with integrated videocard, unless it's something really good.
