BAN Command! - Osviux - 02.01.2011

Could someone make a /ban command that works like this:

An admin writes /ban player and then a GUI List occurs and the admin has to choose a reason for banning and when he does it writes. Player name was banned by administrator name. Reason: choosen reason.
