What does this mean:
[15:12:49] Packet was modified, sent by id: 21, ip: 188.124.214.162:1496
I found it in my server_log.txt? Thanks.

What came from the player (command, text, values, etc) was changed by the player/client. That is in extremely simple terms.
More than likely someone who calls themselves a 'hacker'.

It's basically nothing important at all.
Also @iFriSki you are wrong. This could happen to any player under the right circumstances.

I have seen this happen to players with connection issues..
