No & Yes.
Depends on you, 0.3a is still good, but most servers are starting to update to 0.3b.

No, there are cheat systems which are stable and do work. People just use S0bi3t because they are the only ones people know about

Back to the topic, yes, you should upgrade to 0.3b. It provides vital security fixes as well as fun new features and improvements.
Yes, you should have an anti-cheat system as well. Removing anti-cheat is like removing one of your body's organs - you might be able to live with it, but you'll need to monitor it much more often.

Yes, it is outdated. As ev0lutionnn mentioned, the main purpose of 0.3b was to fix a vital security issue.
